Data Protection Subject Access Request form

The Data Protection Act gives people the right to know what personal information an organisation has about them. To use this right, they can make what is known as a subject access request. This is a 7-page form to be used by people making a subject access request. It describes the information we require, and the process we follow, to respond to a subject access request.
